What to expect at your visit

During the Annual Wellness Visit, your annual wellness specialist or Advanced Practice Provider will:

• Evaluate your current health and complete a health risk assessment
• Check your hearing, blood pressure, height and weight
• Review your prescription medications and supplements
• Answer your health-related questions
• Find out if you're up-to-date on health screenings and immunizations
• Develop a plan to prevent disease and improve your health
