- One-on-one visits are provided by a specially trained annual wellness specialist or Advanced Practice Provider

What to expect at your visit
During the Annual Wellness Visit, your Advanced Practice Provider will:
- Evaluate your current health and complete a health risk assessment
- Check your hearing, blood pressure, height and weight
- Review your prescription medications and supplements
- Answer your health-related questions
- Find out if your are up-to-date on health screenings and immunizations
- Develop a plan to prevent disease and improve your health
