Senior-Focused Health Services at PACE

The Reid Health Pace Center was developed as a dedicated health center tailored specifically for aging adults. Along with providing access to an interdisciplinary team, PACE also will provide the same benefits you would receive under Medicaid and Medicare plus many more if authorized by the Reid Health PACE Team. PACE Health Services

PACE takes the pressure off caregivers by providing access to senior-focused services housed and coordinated under one roof.

PACE Services include:

  • Primary Medial and Nursing Care - includes physical examinations, immunizations, preventive health, consultations, and women's health service
  • Senior-Focused Primary Care - includes audiology, optometry, podiatry, dentistry, and any other care needed in the participant's care plan
  • Nursing Care - includes skilled RN services, assistance with medications, and instructions to prevent illness or disability
  • Home Health & Medical Equipment- includes light housekeeping to personal care and providing assistance with dressing, bathing, and more
  • Personal Care - includes bathing, hair and nail care, dressing, grooming, and assistance with other hygiene activities
  • Medication Management
  • Physical, Occupational, and Speech Therapy
  • Recreational Therapy
  • Social Activities in the Day Center - including a television room with relaxing furniture and a library, creative outlets with mental stimulation and social interaction like arts & crafts, herb gardening, devotions, BINGO, Daily Chronicles & Coffee, and much more!
  • Meals & Nutritional Counseling
  • Social Services
  • Transportation - includes transportation to and from PACE Center as needed, transportation to and from specialty appointments and other services not available at the center, and ambulance services in case of an emergency

After Hours or Emergency Care

Care is available to PACE participants 24 hours a day all year long. If a participant or their caregiver needs to speak to a health care professional to receive advice or treatment and cannot wait until normal PACE Center care hours, afterhours support is available. Additionally, urgent care and emergency care is available for PACE participants if their conditions fall within the definition of an emergency or urgent care situation as identified in the enrollment agreement. Meet Our Care Providers

PACE Center: Meals & Nutritional Counseling

Eligible PACE participants will have a full lunch provided along with morning and afternoon snacks. Nutritional counseling is also a key part of participant care plans. The Reid Health PACE Center provides meals that are prepared for specific dietary needs, and are also fresh, delicious and culturally familiar. PACE participants also receive personalized counseling so they can make informed choices about nutritious meals at home.

Choose Well at Home

Choose Well at Home is a healthy meal delivery program designed to help participants have healthy food in their homes. Meals are prepared by registered dietitians and include at least two food groups and meet the following criteria:

  • Calorie controlled meals
  • Low in saturated (bad) fat
  • Low in sodium
  • Carbohydrate consistent

PACE Center: Home Health & Medical Equipment

Many of the services available at the Reid Health PACE Center are also available within a participant's home. Services will vary depending on needs identified for each participant by their care team. All services are provided with the intention of keeping our participants independent, living safely in their homes.

PACE Home Health Services Can Include:

  • Homemaker/Chore Services and Personal Care
  • Home Health Aide Services or Physicals and Occupational Services
  • Home Delivered Meals (Choose Well at Home)
  • Skilled Nursing Services and Medical Social Services

Reid Home Medical Equipment

As part of the PACE enrollment process, participants will have a home visit where PACE team members will identify medical equipment that a participant will need to live safely and independently in their home. Equipment could include medical supplies and durable medical equipment that is delivered to a participant's home. Learn more about Reid' Home Medical Equipment store.

PACE Center: Medications

To make medication management easier, prescriptions are available at Reid Health PACE Center to make it easy to stay compliant with medications. By housing our participant's medications in one place, the PACE care team can watch for potential drug interactions, dosages, and time between doses to help keep participants safe. Plus, the PACE staff can assist in reminding participants when they're due for a refill.

To make medication management most efficient, Reid Health PACE has partnered with CareKinesis, a PACE-specific pharmacy that focuses on reducing medication-related risks while enhancing economic, clinical, humanistic outcomes.

PACE medication services will provide and coordinate 100% of the needs and management for PACE participants.

Pharmacists work directly with the PACE care team to develop personalize medication regimens that are more safe and effective.

Services include:

  • Medication safety review
  • Monitoring for adverse drug events
  • Dependable medication delivery directly to home
  • Onsite Medication Dispensing Unit
  • Community Pharmacy Network

PACE Center: Social Services

Reid Health PACE Center offers comprehensive social services to help participants maintain not only optimal physical health, but mental and emotional health as well. A PACE Behavioral Health Specialist will help ensure participants and their loved ones have the support they need.
