Why should I fill out the preadmission questionnaire when I get to the office and have to answer all the same questions again once or twice depending on who's asking? Either don't do pre admission questionnaires or FIX the interface of your system!

This was first visit with Dr Hornak and both of us (xxx and xxx his spouse) felt comfortable with the Dr and impressed with his knowledge of xxx's case and the comfortable way he communicated with both xxx and xxx. He was friendly and shared information about things to be done before next visit .

My heart monitor results hadn't come back yet so I kind of felt it was for nothing. Nurse did call later but her results were different than my chart.

I got there early but that's okay the nurse's helped the doctor was very nice and the male nurse was good at what he did

Provider produced good knowledge of my condition and discuss . He was a man that was recommending by a caregiver that I have known for over 50 years. This was discussed prior to my transfer. Both are great doctors.

Thought I was going to see a cardiologist, instead mr.hornak did nothing but belittle me for my smoking tobacco at one point I told him I was there for chest pain he continued with the smoking and how he couldn't figure out why people wants to put smoke into their lungs that he wouldn't put his head into a smoking grill while making burgers told him I wouldn't eat grilled burgers taste to much like smoke then he said you wouldn't run into a burning house and breathe the smoke into my lungs told him I would go in to save someone I finally told him I enjoyed smoking and would stop when I got ready to stop smoking. Dr.Hornack listened to my lungs that was the end of my Cardiologist appointment and you people have him on your payroll must be desperate for help. I will never see one of Reid's specialists again.

I continue to be impressed with the care I get from Reid physicians and Nurse Practitioners, Nurses, etc. Dr. Hornak and staff were so professional and compassionate. I felt completely at ease being under their care.

Had to wait 1 hour to see the doctor when the waiting room was not busy. Ridiculous.

I love the telehealth, it helps me so much with not having to drive to Richmond from Eaton, Ohio, and it is so convenient.

In fact I had concerns about my wife's health and Dr HORNAK set her up with an appointment as a new patient. Reid is fortunate to have a giving and caring Doctor like Dr. HORNAK!!!

My regular cardiologist Dr. Fleming recently cut back on his hours. I appreciate Reid's notification of this and I am content with Dr. Hornak who will be taking over my care.

Valet gentlemen was very kind & helpful. Also, since Dr Flemming has retired we have other family members who has had to decide which Cardiologist they wanted to transfer to. We recommended Dr Hornak but any & all are top notch.
