Dr. Hornak was very kind and took time to talk to me about my heart and health since the valve replacement. He also listened and really cared when I told him I had been through a rough summer with my husband's death. He is very understanding and I would definitely recommend him to others.

Dr. Hornak is a very concerned and caring doctor. He listens to you and doesn't rush you out. Very satisfied with him. Hated to lose Dr. Joseph but Dr. Hornak is very likable and concerned.

Never payed copay before why now.my last visit didn't have to pay copay

The exam room felt like a freezer. The office was pretty busy so I had to wait a bit which wasn't an issue but the room was uncomfortably chilly.

Discussed most my medical problems showed up at the same time (age 70 & older)! This knowledge helps create my plan of care going forward.

My Mom had a great appointment with Dr. Hornak. He took as long as we needed and was very kind and caring. Thank you!

The ?MA ? Nurse that took me to the room was cold. She did not use my preferred name on my chart and never introduced herself. Couldn't pronounce even the simplest of medicines. Pretty embarrassing for a medical office .

Dr Hornack is an excellent provider and a good guy. He's easy to talk with and can relate to me since we're close to the same age.

Why did I have to complete check in process if I checked in on line. Staff at the counter is always extremely rude. The rest of the nursing staff very professional & outgoing.

Dr. Hornak is great! This was my first visit to this particular Reid facility and all went well.

I have something that doesn't seem right. I told him that the medications were ordered for me at the hospital pharmacy. I've gone twice to get my medications, and they tell me they didn't order them. I'm without them right now. Please, if there's anyone who can help me with this, I'd appreciate it.

Prefer using Cambridge City office due to overbooking of patients at Reid Health.

After paying 500.00 for a visit more than listening to my lungs would have been great. Didn't check for edema or anything in my legs. Didn't question previous BP at home since discharge to follow up viisit. didn't question weight gains or anything

There are too many texts/reminders/pre registering/, etc before the appointments. We have to go through all of that when we get there anyway! A reminder a day ahead would be plenty.

It would've been a 10 but nobody's perfect?

Treated me great and cleared me for a medical procedure that I need performed.

The girls that did our intake were so friendly and nice to put me at ease. Dr. Hornak was so knowledgeable and helpful and made me feel reassured!

Dr Hornak was wonderful! He is so nice and friendly and showed great concern for me. His staff was very nice too. When we left we weren't sure about medication directions, supposed to stop one drug and start a new one. A nurse was incorrect as well because she said there were no changes made. We told her we thought there was supposed to be a change. She had to verify her information and give us new paperwork then. That was a bit of a problem communicating information correctly but we were so impressed with Dr. Hornak we would still rated him the highest possible and staff was very kind as well. He is a great asset to Reid. You are lucky to have him and we were fortunate and thankful to be referred to him!

I had a very good experience with everyone I came in contact with at my appointment today!

Informative, friendly

Patients are expected to be on time for their appointments yet then they are forced to sit and wait in a tiny exam room with no windows for 35 minutes for the doctor to appear. An update on when the doctor will arrive would be appreciated before the staff leaves. And an apology or explanation from the doctor only seems appropriate and should be a common courtesy when someone has had to wait.

Waited some fifteen minutes for Dr. Not excessive.

Language I could understand.

The Medical Assistant and doctor were both great.

8:30 am appointment. Waited until 9:15 am to see Doctor.

Dr Hornak was a very kind n compassionate man polite n courteous with a very easy going personality easy to talk too I felt as though I was in good hands

My appointment was at 1:30pm and he did not get into the room until 2:10pm no one coming into tell me that he had an emergency or anything just left me sitting there

I really appreciate the care I received today from my doctor, as well as the nurse who took care of me at the beginning of the appointment.
